Our patient portal is through FollowMy Health. To sign up for the portal you will need to provide our office with your email address to receive the invite. You can call our office or send an email to firstname.lastname@example.org to request the invite. Please make sure to include the email address you would like to use for your account and your date of birth in your email to us. After you have activated your account, you can use the portal link on our website to access your medical records. Here you can request a refill of your medication, email your physician, review test results, request another appointment, pay your bill, and complete required paperwork.
