  • How do I reach my doctor after hours?

    If this is a true medical emergency; please go to your closest ER.  If you are a patient, hospital or physician and have an urgent question please contact our exchange at 888-549-5169. If you have a non-urgent medical question, need to schedule an appointment, need your test results or a refill of your prescription, please contact the office during normal business hours of Monday-Friday 8:00-4:30 pm.

  • How do I schedule an appointment?

    Making an appointment is easy. You can call any of our offices to schedule an appointment or you can request an appointment through our patient portal.

  • Tell me about your Patient Portal?

    Our patient portal is through FollowMy Health. To sign up for the portal you will need to provide our office with your email address to receive the invite.  You can call our office or send an email to to request the invite.  Please make sure to include the email address you would like to use for your account and your date of birth in your email to us.  After you have activated your account, you can use the portal link on our website to access your medical records. Here you can request a refill of your medication, email your physician, review test results, request another appointment, pay your bill, and complete required paperwork.

  • Do you accept Care Credit?

    We currently do not accept Care Credit. Our office accepts cash, debit cards, credit cards (MasterCard, Discover, and Visa) and personal checks.  Contact our billing office with questions: 314-336-5036

  • How can I request a copy of my medical records?

    At this time, please contact our office to request a copy of your medical records.
